Explaining Therapy to Relatives: A Short Script and a Boundary

Someone will ask. At Eid, at a wedding, in the lounge when your child does not say salaam the way the cousin did. Therapy kya hai? Psychologist? Kuch ghalat hai? You have about twenty seconds before the room decides for you. This page is those twenty seconds, and the boundary after them.
It is not the page for a family that refuses to see a problem. That is denial, and it has its own guide: when the family says nothing is wrong. This one assumes you are already going, or have decided to go, and you need language that does not start a trial. Script, then boundary. Not a debate.
This is a script, not a debate
A debate has a winner. A family gathering does not offer you a winner. It offers you an audience, a child who can hear, and a relative who will repeat a shorter version of whatever you said. Your job is to give them a short, true sentence they can repeat, then stop. Extra detail is not honesty. Extra detail is material.
Three rules before any script:
- Talk about the skill, not the label. “Someone is helping him with talking” is true and usable. “We are getting him assessed for autism” is a grenade, and we do not diagnose here anyway.
- Do not argue nazar, taweez, or “your uncle talked at four.” Those arguments are endless and they are not why you came to the wedding.
- The child is in the room until you are sure he is not. If he can hear, the script is even shorter, and it is kind.
You are allowed to be boring. Boring is how this ends.
The 20-second version
Memorise one. Use it until it is stale.
- Talking: “A teacher is helping him with his words. Same as tuition, but for speech. It is going fine.”
- Behaviour / sitting: “He is learning to sit and wait, so school is easier. Short sessions in Multan.”
- Learning: “He needs extra help with reading / writing. We have someone who teaches it slowly.”
- If they push for a name: “There isn’t a big name. We are working on talking / sitting / sleep. I will tell you if that changes.”
- If they push for whose fault: “Nobody’s. These things happen. We are just getting him help.”
- If they say psychologist as an insult: “It is teaching, not a hospital. We go Monday to Friday, like an appointment.”
Then you change the subject. Food, the baby, the cricket. You do not add a paragraph because the silence feels rude. The silence is the boundary starting.
If you want a slightly longer version for one trusted person — a sister, a brother who will actually help — you can say: “He is behind on talking compared with other children his age. We had a look in Model Town. They do not diagnose. They gave us a plan for home. I need you to use the same two words we use, not to fix him at Eid.” That is an ally briefing, not a press conference.
What therapy is not
Relatives fill gaps. If you do not name what it is not, they will name it for you.
- Not a diagnosis. Mahnoor Baloch leads first assessments and describes what she saw. We do not diagnose autism or ADHD, and we do not prescribe. A file from us is not a stamp on a rishta.
- Not a mental hospital. A consultation is Rs 1,500, up to 50 minutes, in a room on MPS Road, Model Town, or on video. Children play. Parents watch. It is closer to a lesson than to a ward.
- Not proof you failed. Children need extra teaching for the same reasons they need glasses. You did not cause this by working, by a phone, by something you ate, or by a fight in pregnancy.
- Not a 40-hour American ABA package. We do not run those. We do not have a BCBA. Noor does behaviour hours on a plan. Intensity of that kind is a different model, in a different country, for a different service.
- Not secret because it is shameful. You may keep it private because it is yours. Privacy is not the same as shame. You can say “we are keeping this in the immediate family” without apologising.
If they want to know what a first hour actually contains, send them what happens in a developmental assessment or take questions to ask yourself so you can answer one precise thing rather than everything.
Boundaries that are allowed
You are not required to give a seminar in someone else’s lounge. Allowed sentences, said once, calmly:
- “I am not discussing his sessions at the table.”
- “Please do not ask him to perform a word for the guests.”
- “If you have a suggestion, tell me in the kitchen, not in front of him.”
- “We are not trying a taweez / a diet / a YouTube cure alongside this. Thank you.”
- “You can ask me. You cannot ask him what the therapist said.”
A boundary is a sentence plus a change of behaviour. If you say “please don’t” and then answer twenty follow-up questions, you taught them that “please don’t” means “ask nicer.” Walk to the other room. Hand the child to the parent who is not in the conversation. Put the phone down. The physical move is what makes it real.
In a joint family you may not control the house. You still control your mouth and your child’s body. You can take him outside. You can leave early. You can skip the gathering that is always a test. That is not cutting off the family. That is refusing an exam.
When they ask in front of the child
Assume he understands more than he can say. That is true for almost every child we see. Do not describe his delays over his head. Do not laugh. Do not say “he is the one who doesn’t talk” as an introduction.
A line you can use while he is there: “He is learning some new words. You will hear them when he is ready.” Then you speak to him, not about him. “Do you want water?” Wait. Carry on with the room.
If a relative grabs his face, asks him to say salaam ten times, or compares him to a cousin, you interrupt. “That’s enough, he has already said it.” You will feel rude. Rude is cheaper than a child who learns that family events are a speech test. We say the same thing about therapy homework: do not make him perform for guests. It makes most children talk less, not more.
What to say at a wedding, Eid, a family dinner
Wedding. Noise, cousins, no escape. Keep him with one adult who knows the plan. If someone asks, use the twenty-second script and go to get food. You do not owe an aunt a developmental history between the mehndi and the dinner.
Eid. The salaam line is the trap. Practise a version he can do — a wave, a hug, “Eid Mubarak” as an approximation — and protect it. Relatives who insist on a perfect pronunciation in a packed lounge are asking for a show. You can say “he has already greeted you” and move on.
Family dinner in the joint house. Seat him next to the adult who will not quiz him. Have one sentence ready for the person who always starts: “We are not doing questions at dinner. Tell me about your trip.” Repeat as needed. You are a broken record on purpose.
If you live at a distance and only see this family twice a year, you do not have to update everyone to the current plan. “He is in a good routine in Multan” is enough. Distance is not a reason to dump a year’s notes into a WhatsApp group at midnight.
The one-page you can show
Some relatives change when they see something written by a professional, not by you. After a consultation we will give you findings in plain words. That page is for you. You may show one person if it helps. You are not required to photocopy it for the family WhatsApp.
What to show, if you show anything: the skill, the next step, the home line. What not to show: guesses about the future, comparisons with other children, anything that reads like a verdict. If a relative wants “the report,” ask why. School sometimes needs a short letter. A cousin does not.
If you already have a thick file from another city and relatives are arguing about what it means, that is a second opinion for you and us — not a family reading group. We will not re-score a test we did not give. We will say whether the plan matches this child.
If they keep offering “cures”
Honey, almonds, dropping English, more television in English, less television, a peer who will “make him talk,” a pir, a diet, a heavy metal test, a 40-hour centre in another city. Thank them. Do not take the bait of explaining the evidence in the lounge. “We have a plan with a speech therapist in Model Town. We are sticking to it for three months. I will ask you if we need ideas.” Then stop.
If they book something behind your back — a taweez on the child, a visit you did not agree to — that is a boundary with the adults, not a therapy problem. Be direct once: “Do not take him for treatments I have not agreed.” If it continues, he does not go to that house alone. That sounds stark. Children get put through a lot in the name of being helpful.

You do not owe a diagnosis to the group chat
Mute the thread. Answer one person by private message if they are actually useful. “Please do not discuss him in the group” is a complete sentence. If they do it anyway, leave the group for a month. The child will not suffer. You might sleep.
Fees, hours and location are not secrets, and they stop some of the wilder rumours: Rs 1,500 for a first 50 minutes, Monday to Friday 2pm to 6pm, Model Town, +92 301 6424265. Send fees if money is the gossip. Send online versus in person if the gossip is that you are “not even going.” Send nothing if the gossip is just gossip.
Families who travel from Lodhran, Bahawalpur, Sahiwal get a second round of questions — why Multan, why not locally, why the drive. “This is the team we chose” is enough. You do not have to justify a road.
The aim is not to make the family understand. The aim is to keep the child out of the argument, keep the plan intact, and keep enough of a relationship that Eid is still possible. Twenty seconds. Then a boundary. Then you go and get the tea.
Frequently asked questions
What do I say when an aunt asks if something is wrong with him?
A twenty-second script about the skill, not a label: “A teacher is helping him with his words. It is going fine.” Then change the subject. You do not owe a diagnosis, and we do not diagnose here anyway. Extra detail becomes material for the group chat.
Should I tell relatives it is a psychologist?
Only if it helps. “Teaching for talking / sitting / reading” is true of most of what we do and starts fewer fights. If they spit the word psychologist, you can say it is a lesson, not a hospital. Muhammad Salman Afzal does psychology when feelings and family load are the work — still not a ward.
They ask him to say words in front of guests. How do I stop that?
Interrupt once, clearly: “He has already said it” or “We are not doing performances.” Then move him or yourself. Asking a child to perform speech for relatives makes most children talk less. Rude is cheaper than a test every Eid.
Do I have to share the assessment report with the family?
No. The page is for you. You may show one trusted person the skill and the next step if it helps. A cousin does not need the file. School sometimes needs a short letter. That is different from a WhatsApp broadcast.
They keep sending YouTube cures and taweez suggestions.
Thank them, name the plan you are sticking to for three months, and stop explaining the evidence in the lounge. If someone takes him for a treatment you did not agree to, he does not go to that house alone. Helpful relatives still need a boundary.
Is this the same as when the family says nothing is wrong?
No. That page is denial — they will not agree there is a difficulty. This page is for when you are already going and need a script plus a boundary so gatherings do not become a trial. Use both if you have both problems.
