Getting to know your child
Fill this in before we meet — in Multan or online — and we already know a little about your child before the appointment even starts. That means we spend our time together on what matters. It takes about ten minutes. There are no right or wrong answers here, so leave anything blank, or write “not sure”, whenever that’s the honest answer.

💛 Before you start, take a breath. Fill in only what you can. Please leave anything blank that hasn’t happened yet, doesn’t suit your child’s age, or you’re simply not sure about. That’s completely normal, especially for babies, for older children, and for children who already have a diagnosis. Nothing here is compulsory, and nothing here is marked.
Short on time, or filling this in for a grandchild? Sections 1 and 2 on their own are plenty. Do it before your visit if you can, or just bring it along with you.

💡 If yes, do skip sections 4 and 5 and just tell us, in your own words, what’s going well and what’s hard. And if you have any reports to hand — a diagnosis letter, hearing or vision results, old therapy or school notes — bring them along or send us a photo. They genuinely help.
